Student Emergency Medical Forms
Allen East uses FinalForms for digital forms. Students in grade 5-12 are required to provide a digital signature as well for a few of the forms. Once parents have completed the forms, the student will receive an email on their Gmail account. The username is exactly the same as students use to log into the Chromebooks. Make sure students have @student.alleneastschools.org on the end of their email address.
